A Donegal woman says she wouldn’t be walking on her own without the help of the ICPOP team.
There are 30 ICPOP teams across Ireland who help older people live at home and avoid unnecessary hospital visits.
They also support people in returning home after a hospital stay, offering up to six weeks of care in the community and at home.
