A US Secret Service internal review has found there were multiple security failures on the day of the first assassination attempt on Donald Trump.
The report identified poor planning and communication breakdowns, as well as ‘complacency’ among some agents, at the Pennsylvania rally.
The report also highlighted the failure to secure the building the shooter fired from.
